Yeah I only realized quite recently that when Americans say sausage on pizza, they're talking about uncooked sausage meat without casing. I always though sausage was another a regional term for pepperoni.

Sausage is always a gamble because different places use different sausage. Ideally I'd want both on my pie with black olives, green peppers, and mushrooms.
Pebberoni. The "sausage" most pizza shops use is foul Jimmy Dean sosig roll shit. Pebberoni is great either, its usually too salty and greasy. Cheese or a couple of vegetable topppings is the best choice unless you're going to an upscale pizza joint where they have high quality meat.
Depends on the quality/flavor of sausage. In that pic, I'd prefer sausage because it's an evenly distributed mince so the flavor in each bite would be more consistent than the pepperoni.
