I knew a guy who was a health inspector and he said the only time he ever saw a live rat in any kitchen was a Fazoli's

That health inspector wasn't on the job very long. I've worked in numerous kitchens throughout Chicago. They all have some combination of mice/roaches/rats.

These have started popping up everywhere here.

No drivethroughs but they're designed for takeaway and there are a few attached to petrol stations.

If you're talking about fast dine-in service like taco bell then you have a valid question. Some people will try to answer your question by saying olive garden has a to-go section, but that didn't become popular until after coronavirus. Italian food in America has always been seen as fancier food that you should dress up for and eat inside. There's a local spot down my road where the customers are usually higher class, but they also have seen a spike in pick-up orders after coronavirus. Historically the menu and kitchens weren't designed to provide fast service until Fazoli's found out how. Most American's don't say "I NEED ITALIAN FOOD NOW FROM THE WINDOW!", drive-thru has always been more of a burgers and fries thing.

      >Italian food in America has always been seen as fancier food
That's French food, Italian food ain't that fancy.

French food is seen as actual fancy food, and as a result most people don't go out to eat French food.
Italian food is approachably "fancy." It's something that a lower middle class family will go to, but they are very likely to dress up a bit.

How do you eat pasta while you're driving? Burgers and burritos make sense.

The only Italian food you could eat while driving would be calzones.
